If your organization uses the Safety Management System, you can report safety events, hazards and concerns from Flight Circle. Your reports are reviewed by your organization’s safety personnel.
Submitting a report
- Go to Safety > Submit Report.
- Enter a Title and a Description of the event. Both are required.
- Optionally choose a Category, enter the Location and Event Date and Time, choose the Aircraft and set the Severity (Low, Medium, High or Critical).
- Optionally attach up to 5 files of 25 MB each. Photos, PDFs, Word, Excel, CSV and text files are accepted.
- Optionally add witnesses with their name and phone number.
- Click Submit Report.
Location data is removed from attached photos.
Reporting anonymously
Check Submit anonymously to hide your identity. Nobody at your organization, including administrators, can see who submitted an anonymous report.
Anonymous reports have some limits.
- You will not receive an email when the report is resolved.
- ASRS details cannot be added to the report.
Following up on your reports
Your Submissions, at the bottom of the Submit Report page, lists your reports with their severity and current status. When a report you submitted is resolved, you receive an email with the resolution.
Filing with NASA ASRS
Click ASRS Details next to a report to fill in the details for a NASA Aviation Safety Reporting System report. Then click Export for ASRS to get a summary to copy into NASA’s online form. Your personal identification is not included in the export. Enter it directly on the NASA form.
