Go to Safety > View Reports to see all safety reports submitted to your organization. You need the Safety Reports permission set to View & Add Notes or higher.
Finding reports
By default the list shows reports that are Open or Under Review. You can filter by status, category, severity and event date range. The list shows the date, title, category, severity, status, reporter and assigned person.
Click Export CSV to download the reports that match your current filters.
Report statuses
- Open is the status of every new report.
- Under Review means the report is being investigated.
- Resolved means the investigation is finished. The person who resolved the report and the date are recorded.
Severity is set by the reporter as Low, Medium, High or Critical. Reviewers can change it.
Working a report
Click a report title to open it. From the report you can:
- Change the status, severity and category.
- Assign the report using Assigned To. The list contains the account owner and members of groups whose Safety Reports permission is View & Add Notes or higher.
- Upload attachments and add witnesses.
- Add notes. Notes are internal. Only users with Safety Reports permission can see them, and notes cannot be edited or deleted once added.
- Write a resolution and click Save Resolution.
- Click Download PDF for a printable copy of the report, including internal notes.
- Click Create Notice to publish a Safety Bulletin notice pre-filled from the report.
Resolving a report
- Enter the resolution and click Save Resolution.
- Change the status to Resolved.
When the status changes to Resolved, the reporter receives an email with the resolution. Save the resolution before changing the status so it is included in the email. Anonymous reporters do not receive this email.
Anonymous reports
Members can choose to submit a report anonymously. The reporter is shown as Anonymous everywhere, including the report list, the report itself and the email notification. Nobody at your organization, including administrators, can see who submitted it.
Email notifications
When a new report is submitted, an email is sent with the title, category, severity, event date, reporter and a link to the report. By default it goes to super administrators and the account owner. To choose who receives these emails, see Report Settings.
